Annual review with a GP

The needs of someone with dementia can change with time.

An annual review is key to support a person with dementia and an opportunity to review their care.

Following a diagnosis of dementia, the patient should be invited by their GP practice for a review of their dementia care on an annual basis.

At the review, a health care professional (for example a GP, nurse, or care co-ordinator) will discuss any patient or carer's concerns regarding any health and social needs, review medication, check for new symptoms or changes in behaviour, and discuss planning ahead and support for carers.

The outcomes from the review can be summarised in a personalised care plan.
