Community support if you’re socially isolated
The Prevention Matters service supports individuals who are socially isolated, or carers, to connect with their community, build social networks, engage with formal services and reconnect with friends and family.
Eligibility criteria
- aged 18+
- feeling isolated and/or a carer
- living in Bucks or registered with a Bucks GP
Prevention Matters support

- improving health and wellbeing
- linking with the local community
- remaining active
- increasing confidence
- attending a range of activities
- linking with local services
The team
The Prevention Matters Team consists of several Community Practice Workers (CPW). Each CPW works within a particular local area and has lots of knowledge about what is happening in that local area.

If you would like to now more about the referral form you can call 01296 484322.
Once you have sent the referral form back, the team will be in touch within 48 hours to discuss arranging an assessment. Referrals can be made by other agencies, your carer or yourself.
During the assessment, the team will have a chat with you about what you would like to gain from the service and look at how they can support you. This normally takes place within ten days from the initial referral.
If this is the right service for you, you will be assigned a CPW who will visit you up to six times, over an eight-week period to help you to achieve your goals.
If your needs change during the support period, your CPW will work with you to find the right place for support.
Digital support
Prevention Matters can also help you to learn new digital skills, access the internet and to increase your support networks. You can discuss this with your CPW at any point in the support period.
