What happens if you are admitted

Buckinghamshire Healthcare NHS Trust is an integrated service providing care and treatments in a variety of settings including in a person’s home.

If they know that you have a formal diagnosis of dementia, they will ask if you or your family would like to complete a personal passport called ‘This is Me’ if you do not already have one.

This will help them know a little bit about your preferences and your personal history so they can adapt your care accordingly. For example if your preferred drink is tea with sugar and you cannot tell them, the This is Me passport will make sure they give you tea and not coffee.

If your family would like to be part of your care, they can accommodate this through John’s Campaign.

Services

All BHT services try to be dementia friendly so will plan your care to make sure you know where the toilet is and feel occupied during the day.

If you have a suspected diagnosis of dementia they will continue to assess for clinical symptoms to help reach that final diagnosis and will navigate you and your family to local services.

Power of attorney

If there is a Power of Attorney in place a copy will be asked for to ensure the trust is making decisions correctly for you following legal processes.
