Dementia diagnosis and support

Community support if you’re socially isolated

The Age Better Bucks service is for older adults (65+) who want to maintain their independence and wellbeing.

Eligibility criteria

The service is for people who:

  • are aged 65 and over
  • live in Buckinghamshire
  • would benefit from preventative support to stay well and independent

They're unable to support clients who:

  • are currently receiving a package of care from Buckinghamshire Adult Social Care
  • are residents in care homes or supported living
  • have already accessed the service in the current calendar year

What the service offers

The service includes a visit from a friendly member of ageUK staff who will talk to you to try and understand your situation and your goals.

From that conversation, they help create a personalised plan which could help you to:

  • build connections with other people or groups
  • adapt to changes in your health or mobility
  • cope with bereavement
  • get online and use technology
  • build new routines
