How we will work

To deliver this strategy we know we need to work differently.

Working with our communities

We are committed to working in and with our communities to understand the needs and wants of our communities. This will mean people are more closely involved in developing services to ensure we make the best use of all our local assets and resources.

Joining up care

We will work together to plan and coordinate care that has the person at the centre. This will mean people will experience more seamless care that is personalised to them and will be able to more easily navigate the services they require.

Evidence led

We will use data and community insights to support care and service improvements, ensuring we are able to target improvements where we will get the biggest benefit for our population.

Equity

We will seek to address Buckinghamshire’s health inequalities in all of our work and use targeted programmes such as Opportunity Bucks to help us achieve this. We will develop a set of metrics which will be included in our Health and Wellbeing dashboard and will be held accountable through the Health and Wellbeing Board so that we can measure our impact.
