Examples of the process you may experience

The below refers to persons who:

  • is worried about their memory
  • has a dementia diagnosis
  • is the carer of someone with dementia/memory concerns

Individual/Carer A - No concerns

  1. The individual/carer contacts their GP due to memory concerns
  2. The GP practice arranges initial assessment (which includes blood test and memory screening)
  3. The GP finds no concerns following investigations
  4. Individual/carer does not need to progress further on the pathway

Individual/Carer B - Diagnosis made and needs support

  1. The individual/carer contacts their GP practice due to memory concerns
  2. The GP practice arranges initial assessment (which includes blood test and memory screening)
  3. The GP makes diagnosis – no further testing is required to determine diagnosis
  4. The GP practice advises person to contact Buckinghamshire’s Dementia Support Service (DSS) on 0333 150 3456 for post diagnostic information

Individual/Carer C - Referred to memory clinic

  1. The GP will assess whether further investigations are required including specialist input regarding dementia medication
  2. If further investigation is required the GP will refer the individual/carer to the Memory Clinic who will then arrange additional tests
  3. Memory Clinic will make a dementia diagnosis (and dementia medications will be considered)
  4. Memory Clinic will advise the individual/carer to contact Buckinghamshire’s Dementia Support Service (DSS) on 0333 150 3456 for post diagnostic support
  5. If the individual/carer agrees to receive post diagnostic support from Buckinghamshire’s Dementia Support Service (DSS), the GP / Memory Clinic will put forward a referral to the service on their behalf
  6. Buckinghamshire’s Dementia Support Service (DSS) will make contact with the individual/carer and provides ongoing post diagnostic support based on need until required (there is no discharge policy)

Individual/Carer D - No support needed from Dementia Support Service (DSS)

  1. The individual/carer contacts their GP practice due to memory concerns
  2. The GP practice arranges initial assessment (which includes blood test and memory screening)
  3. The GP will assess whether further investigations are required including specialist input regarding dementia medication
  4. If further investigation is required the GP will refer the individual/carer to the Memory Clinic who will then arrange additional tests
  5. Memory Clinic makes diagnosis (and dementia medications will be considered)
  6. Memory Clinic advises person to contact Buckinghamshire’s Dementia Support Service (DSS) on 0333 150 3456 for post diagnostic information
  7. If the individual/carer chooses not to receive support via Buckinghamshire’s Dementia Support Service (DSS) they are provided with details of how to self-refer into the Dementia Support Service (DSS) at a later date and are also provided with information on how to access support from Social Prescribers who can signpost individuals/carer’s to appropriate services depending on need.
