What to do, where to go and what happens
I have a memory concern/memory has deteriorated
- the first thing to do is to contact your GP practice
- your GP practice will organise an initial assessment and may refer you to a memory clinic for further assessment
- if you have not received a dementia diagnosis, you can still access support via our Dementia Support Bucks (DSB), which provides ongoing support

I have just been diagnosed with dementia
- If you have been diagnosed following assessment at a Memory Clinic or GP practice, you will be offered post diagnostic support by Dementia Support Bucks which can be accessed through a referral on their website.
- See below for the expected response times of the service
- Crisis Point/Urgent Invention required 2 days
- Complex needs/outcomes 4 days
- One to One support required 6 days
- Home Visit required 6 days
- Local Telephone Support required/requested 6 days
Dementia Support Bucks will offer to support you directly and/or refer you to other services.
I have had a diagnosis/needs have changed/more support
The first thing to do is to make a self-referral to Dementia Support Bucks through their website, who can then offer the appropriate support to meet your needs.
Eating balanced meals that include fibre and drinking fluids throughout the day helps to maintain bladder and bowel functions and lower risk of urinary tract infections.
