Learning disabilities and dementia

The Buckinghamshire Community Learning Disability Health Team (CLDHT) is a service that provides NHS support to adults with a Learning Disability.

The eligibility criteria for the service are:

  • the person needs a diagnosed learning disability
  • the person must have a health need that cannot be met by regular NHS services
  • the person must be registered with a GP based in Buckinghamshire

How they can help

The CLDHT team consists of Learning Disability Nurses (both community and intensive support), Occupational Therapy, Physiotherapy, Speech and Language Therapy, Psychology, Psychiatry, Dietician and our Positive Partnerships Team.

Diagnosis

If someone is diagnosed with dementia, the team also offers a treatment and support pathway to help ensure the person’s needs are met as effectively as possible, with an aim to maximise quality of life and allow them to stay in their own homes wherever possible.

Review

A review consists of a series of questions designed to identify any presenting health needs associated with dementia.

Advice and support will be offered around any need including signposting/referral to other services if needed.

If the person remains clinically stable for a period of time they may be discharged to the care of their GP by mutual agreement, but they can be re-referred to the team for review if their needs subsequently change.
